HIQUA to investigate Halappanavar death

The Health Information and Quality Authority says it will investigate the care and treatment provided to Savita Halappanavar, who died at a Galway hospital last month.

It follows a request from the H-S-E, who are conducting a separate investigation.

The investigation will assess whether the services provided at University Hospital Galway complied with national standards, and international evidence of what is known to achieve best outcomes.

HIQA says it’s requested – and received – information from the hospital and the H-S-E, in relation to Mrs. Halappanavar’s case.

The terms of reference and membership of the investigation team will be published when they’re finalised.
